At The Cat Doctor, we are always happy to meet and welcome new clients and patients into our cat care family. We understand how important it is to find the right veterinarian and support staff and will do our best to make you feel comfortable with your decision. As a new client, we want to make sure that your first visit—and every visit after that–exceeds your expectations and is a truly positive experience. Please don’t hesitate to reach out to us with any questions you have regarding our practice or our services.

To help us learn as much as possible about your cat and his or her medical history, please obtain any previous health records that you might have. Or, if you prefer, just notify us and we can obtain the records for you. Please also fill out the following forms so that we can properly prepare your cat’s medical record in advance of your visit:
